研 究

本講座では、徳島大学病院と阿南医療センターの診療を通じて、基礎および臨床面での各診療領域での病態解明と治療の開発研究を行うことを大きな使命としています。研究テーマとしては以下のものが進行中であり、研究論文発表も行いました。

糖尿病・内分泌代謝内科学分野

(1)病態モデル動物及び臨床バイオマーカーの解析と糖尿病・代謝疾患関連合併症の発症機序解明

遺伝子改変動物を用いて、代謝・循環改善薬の介入による代謝変化や動脈硬化症の予防効果の検証や病態解明を行う他、受診患者さんの臨床サンプルや臨床データを解析することで、効果的な治療法の開発を目指しています。

① インスリン抵抗性関連疾患群におけるインスリン受容体切断の意義
インスリン作用が阻害されるインスリン抵抗性は、臨床医学的に糖尿病の病態を担う重要な要素であることが広く認められています。さらに、糖尿病との関連が深い疾患においてもインスリン抵抗性がそれぞれの病態に関わるとされていますが、その基盤となる共通の分子機構は見出されていません。これまで我々は独自の視点で新たなインスリン抵抗性メカニズムの同定とバイオマーカーとしての可溶性インスリン受容体の意義について検討してきており、糖尿病や妊娠期に認めるインスリン抵抗性に共通する分子基盤であることを見いだしてきました (Yuasa et al. BMJ Open Diabetes Res Care. 2021 Dec;9(2):e002467. )。現在、この分子機構が癌やアルツハマー病・肥満・肝疾患といったインスリン抵抗性関連疾患群に広く関わっている可能性を検証しています。

② 糖尿病患者の非アルコール性脂肪肝疾患におけるヘパリンコファクターⅡ(HCⅡ)の意義
近年、非アルコール性脂肪肝疾患(NAFLD)の病態が注目されており、肝硬変や肝臓癌への進行が大きな問題になっており、NAFLDの罹病率は特に糖尿病患者で高率であることから、その進展や防御に関わる因子の解析は、重要な臨床課題となっています。我々は、動脈硬化抑制効果やインスリン抵抗性抑制効果を有するHCⅡが、糖尿病患者におけるNAFLDの進展予防に寄与するという仮説を立て、305例の患者を対象に検証し、英語論文として誌上発表致しました（Hara et al. J Atheroscler Thromb. 2023 Aug 1;30(8):871-883.）。

③ 慢性腎臓病と自然免疫の関係
（基礎）　慢性腎臓病の発症要因となる腎組織における自然炎症反応の亢進には、自然免疫を担当するミエロイド系細胞のオートファジー障害が関与するとの仮説に則り、ミエロイド系細胞特異的にオートファジー機能異常を来す遺伝子改変マウスでの解析を進めています。当該マウスに脂肪酸結合アルブミンを投与する尿細管障害モデルを解析し、自然免疫担当細胞におけるオートファジーは、脂肪酸負荷による尿細管障害を和らげる役割を果たしていることを見出しています。現在、オートファジーによる炎症性タンパク質の分泌制御機構を解析し、論文投稿の準備中です。
（臨床）　SGLT2阻害薬の大規模臨床試験の結果では、糖尿病性腎臓病（DKD）症例の蛋白尿減少効果が認められており、レニン・アンギオテンシン系阻害薬治療下でも、SGLT2阻害薬が蛋白尿を減少させる可能性が示されています。 SGLT2阻害薬が抗炎症効果を発揮し蛋白尿減少効果をもたらすことを見出し、その結果を英語論文として誌上発表致しました（Otoda et al. Diabetes Ther. 2024 Jan;15(1):127-143）。ミトコンドリア由来活性酸素は糖尿病の様々な病態に関与していると考えられています。現在は、2型糖尿病患者に対するイメグリミンとエンパグリフロジンの抗酸化ストレス作用を解析しています。

④ AGE Readerを用いたAGEs指標と糖尿病関連腎臓病の関係
糖尿病疾患などで加速的に生成が亢進されるAGEs：最終糖化生成物（Advanced Glycation Endproducts）は様々な糖尿病関連臓器合併症に関与することが知られています。 我々は非侵襲・短時間にAGEsを測定するAGE Reader muを用いて、蛍光分光方式で、皮膚・皮下の血管壁に蓄積されるAGEs をAutofluorescence（AF/自己蛍光量）として検出し、 その数値が2型糖尿病患者の腎尿細管障害を反映することを英語論文として誌上発表しました (Yamagami et al. Biomedicines. 2023 Nov 10;11(11):3020. doi: 10.3390/biomedicines11113020.)。

(2)生活習慣病患者における早期動脈硬化病変の検出と治療介入の効果検証

受診患者さんの早期動脈硬化症の病態検出目的と心血管合併症発症や予後予測の強力なエビデンスを有する血管内皮機能検査(FMD)の積極的な導入を行い、FMDの正常化を目指した病態解析や治療法の開発を目指しています。我々は、セミオートマチックのFMD測定装置を阿南医療センターと徳島大学病院に導入し、血管内皮機能の改善や悪化に関与する臨床指標の解析を行っており、FMD検査の結果を患者さん個々の診療にフィードバックすることで、生活習慣是正に活用しています。またFMDによる腎機能予後予測に関する検討を行い、英語論文として誌上発表しました (Masuda et al. J Atheroscler Thromb. 2023 Nov 1;30(11):1727-1741.)。

(3)体組成解析を用いた骨格筋代謝および機能異常症における病態解析

高齢化社会において、フレイルは大きな臨床課題となっています。そこで主として糖尿病患者を対象に、骨格筋代謝および機能異常症としてのサルコペニア・プレサルコペニア・ダイナペニアに関わる臨床指標の診断のために、InBody S10®︎による生体電気インピーダンス法(BIA：Bioelectrical Impedance Analysis）を用いた体組成解析を行なっています。日常診療においてBIA法の検査結果レポートを用いた患者指導を行ない、フレイル予防に注力しています。また骨格筋異常のみならず、今回BIAで測定した位相角と体外細胞水分比が糖尿病患者におけるHgbおよびHctに独立して相関することを見出し、英語論文として誌上発表しました (Hori et al. Heliyon. 2023 Mar 21;9(4):e14724. doi: 10.1016/j.heliyon.2023.e14724. )。
また副腎アンドロゲンであるDHEASが、男性2型糖尿病患者の骨格筋機能異常に関して抑制的に関与することを見出し、英語論文として誌上発表しました(Yasui et al. Metabolites. 2023 Nov 3;13(11):1129. doi: 10.3390/metabo13111129.)。

血液内科学分野

(1)各種疾患における口腔内免疫・細菌叢と各種臨床指標の相関に関する検討

近年、常在細菌叢と各種疾患との関連が注目されている。口腔内には多数の常在菌が生息していると同時に、唾液が消化液として分泌されるものの抗菌物質を多数含んでいることが知られています。このような常在細菌叢や唾液中の抗菌物質と、各種疾患または病態との関連はほとんど知られていません。本研究では、当院で診療中の患者において唾液を採取し、各種臨床指標やその経過に如何に影響を与えるのかを検証しています。

(2)造血器疾患における病態関連因子およびバイオマーカーの探索

造血器腫瘍において、化学療法その他の再発例や初回治療療法抵抗例などは依然予後不良であるため、病態に関連する因子の抽出は喫緊の課題です。また非腫瘍性疾患も多く存在し、ステロイドなどの免疫抑制療法が試みられますが、治療反応は個体差が大きく、事前に予測が可能なバイオマーカーの同定も求められています。造血器疾患の患者の検体を収集し、徳島大学血液内科の検体と併せて、新規の病態関連因子や治療の反応性予測因子、および予後因子を見いだす検討を行っています。

(3)新型コロナワクチンの有効性・安全性の検討

新型コロナウイルス感染症が世界的に流行し、人々の生命や生活、経済を脅かしている。抜本的解決策のひとつとして、ワクチン接種が世界中で開始されたが、臨床応用された世界初のmRNAワクチンであり、その有効性、安全性は十分には知られていません。新型コロナウイルスに対する抗体および製剤に含まれるポリエチレングリコールに対する抗体やその持続期間、またCOVID-19感染予防効果、ワクチンの副反応について検討中です。

循環器内科学分野

臨床医として強い問題意識を持ち、ヒトでの臨床研究を推し進め、臨床での問題点を基礎研究により理論的に解明していくという姿勢で、不整脈を中心とした循環器領域の基礎。臨床研究を行っています。

(1)慢性炎症としての心房細動の病態解明と治療法開発

複数の凝固因子（プロテアーゼ）が血栓形成のみならず、さまざまな細胞に発現するプロテアーゼ活性化受容体（PAR）を介して、炎症反応にも関与することが知られています。なかでもPAR-2は主にXa因子（FXa）によって活性化されることが報告されています。我々は、FXa-PAR2経路が心房細動発症に寄与していることをPAR2欠損マウスを用いて生理学的・生化学的に証明しました。さらに、PAR-2のリガンドである経口Xa阻害薬が抗炎症作用を介して心房細動発症を抑制しうることを自然高血圧ラット用いて証明しました（論文掲載予定）。また、近年、動脈硬化などの基盤として慢性炎症が関与していることが知られており、なかでも病原体センサーとしてのToll-like 受容体（TLR）の関与が示唆されています。心房細動の基盤としても慢性炎症が関わっているとされていますが、TLRの関与などその詳細は不明のままです。そこで、我々はTLR9欠損マウスを用いた基礎的アブローチ、心房細動アブレーション症例のサンプルを用いた臨床的アプローチの両面により、TLR9シグナルによる心臓リモデリングと心房細動発生メカニズムの解明を進めています（研究成果の一部は学会・論文として発表）。

(2)心房細動発症の新たな予測因子の検討

心房細動は頻度の高い不整脈でありその予防対策が肝要でありながら、簡便で有用性の高い予測因子はありません。我々は、臨床背景、ホルタ―心電図指標、心エコー指標を総合的に評価した複合スコアにより心房細動発症の予測が可能かを検討しています。現在までの検討では、年齢、上室性期外収縮1日総数、最大RR間隔、左房径が独立して心房細動発症のリスクを高めることが判明し、これらの因子を点数化したスコアが高くなるにしたがって心房細動発症の発症率は高くなることを見出しました（論文にて発表）。さらに、12誘導心電図から右・背中側誘導を算出した導出18誘導心電図という新技術を用いれば、既存の12誘導心電図のP波より心房の状態を正確に推測できる可能性があると推察されますが、過去にそのような報告はありません。そこで、我々は導出18誘導心電図のP波と心房細動発症や左心房サイズとの関連を検証しています。

(3)心房細動・心不全患者におけるサルコペニアと各種臨床指標の相関に関する検討

高齢者に見られる病的な筋肉量の低下はサルコペニアと称され、生理的予備能が低下するフレイルの状態につながるとともに、認知症や、心血管病、腎障害、貧血、免疫力低下などの疾患と密接な関係があることが明らかになってきました。しかしながら、サルコペニアと心房細動の発症・進展、認知症、心不全との関連については十分に解明されていません。そこで、心房細動、心不全にて診療中の患者における体組成測定を行うことで、サルコペニアの頻度の調査を行い、骨格筋量が各種臨床指標やその経過に如何に影響を与えるのかを検証しています。成果は国際学会でも発表し、現在英語論文の投稿準備中です。

放射線医学分野

(1)画像診断ナショナルデータベース実現のための研究開発

日本医学放射線学会は、放射線医学が国民の健康と福祉の増進に寄与するためにICT（情報通信技術）化を推進し、ビッグデータや人工知能を活用した医療の構造改革を実施しています。その一つとして2016年より日本医用画像データベース（J-MID）が始動していますが、2021年からは徳島大学も北海道大学、愛媛大学とともに新生J-MIDに加わり、現在は全国の主要16医療施設の新体制で大規模画像診断データベースの構築を行っています。画像診断検査装置や検査オーダ―、撮影、診断のそれぞれに応じた医療技術、医療安全、医療被ばく低減、医療の質の向上を目的とし、人工知能の開発や発展のための研究開発を目指しながら、現在も全国的な画像データベースの構築を進めています。

(2)テクスチャ解析を用いた卵巣腫瘍や軟部腫瘍のMRI画像およびPET/CT画像の検証

卵巣腫瘍や軟部腫瘍の術前に良性・境界悪性・悪性など組織型の画像診断の精度には限界があり、新たな画像診断法の開発が望まれています。本研究では徳島大学病院産婦人科と整形外科と共同で、術前のMRIやPET/CTなどの画像診断において、画像の質感を数値化できるテクスチャ解析やダイナミックPET/CTなどの新たな撮像法を用いることで、卵巣腫瘍や軟部腫瘍の術前診断精度の向上が図れるかどうかを検証することを目指し、症例登録およびデータ収集を行っています。
